Er:YAG laser-assisted periodontal regeneration with modified minimally invasive surgical technique (M-MIST) under microscope in an advanced periodontitis case with severe attachment loss beyond the root apex

The clinical case involved a 55-year-old patient with a 9 mm periodontal pocket and grade II mobility in the upper left second premolar. Bone loss extended beyond the root apex and was managed using the M-MIST technique combined with the LiteTouch™ Er:YAG Laser.

 

The results: After one year of treatment, the PPD was reduced from 9 mm to 3 mm, the clinical attachment level improved from 10 mm to 5 mm, mobility was eliminated, and the intrabony defect showed near-complete resolution.

 

The conclusion: The use of the Er:YAG Laser with M-MIST, under magnification, proved to be effective and safe for debriding deep periodontal defects. It is a promising alternative in cases where extraction was once considered the only option.
